Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Homecare Nurse - Clinical Care Specialist. This rewarding role involves providing personalized nursing care to patients in their homes, working collaboratively with our interdisciplinary team to ensure coordinated and comprehensive care delivery.

Key Responsibilities
  • Assess patient needs and develop individualized care plans under the supervision of a Registered Nurse (RN)
  • Administer medications, treatments, and therapies as prescribed by the physician
  • Provide patient and family education on healthcare management, medication administration, and self-care techniques
  • Maintain accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies
Requirements
  • Graduate of an accredited school of nursing
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
  • One year of pediatrics experience (South Carolina), one year of experience working under current nursing license (California), or six months of recent experience as a Licensed Nurse in a clinical care setting
