Job Title: LPN Supervisor

Meadowcrest at Middletown is seeking a highly skilled LPN Supervisor to join our team. As a key member of our healthcare leadership team, you will be responsible for providing overall leadership and management of the health and well-being of our residents.

Key Responsibilities:
  • Maintain effective communication with healthcare providers regarding recent changes or requests for orders.
  • Ensure compliance with federal, state, and local laws and regulations related to clinical care.
  • Collect and report occurrences, and assist in developing/implementing improvement plans as needed.
  • Partner with the Administrator and other team members to maintain resident and team member health and safety.
  • Collaborate with the pharmacy consultant to provide optimal pharmaceutical services to residents.
  • Oversee infection control practices in the community.
  • Ensure the safe and efficient distribution/administration of medications, injections, nebulizers, and moderate wound care in accordance with state regulations.
  • Identify ongoing needs and services of residents through the evaluation/Service Plan process, including documentation for residents with a change in healthcare status.
  • Manage the completion of assigned evaluations.
  • Partner with other department coordinators to identify, review, and discuss potential changes in service/medication levels and update the resident's Service Plan as indicated by state regulations.
  • Maintain communication with residents' families and/or responsible parties regarding changes in care or health concerns.
  • Perform other related duties as assigned by management.
Supervisory Responsibilities:
  • Directly supervise certified nursing assistants' employees within the nursing department(s).
  • Carry out supervisory responsibilities in accordance with the organization's policies and applicable laws.
Qualifications:
  • Associate's Degree (AA) or equivalent from a two-year college or technical school, or six months to one year related experience and/or training, or equivalent combination of education and experience.
  • Certificates, licenses, and registrations required:
    • Maintain current state license as a Licensed Professional Nurse or a Professional Registered Nurse per state regulations.
  • Other skills required:
    • Experience in assisted living, home health, or long-term care industries.
    • Two (2) years' experience as a Licensed Nurse.
    • Must demonstrate competence in assessment skills, injections/medication administration, follow-up, and triage.
    • Ability to communicate effectively, speaking the primary language of the residents.
    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
    • Ability to write routine reports and correspondence.
    • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
    • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form; ability to deal with problems involving several concrete variables in standardized situations.
