STAT Home Health is seeking a dedicated and compassionate Physical Therapist to join our patient care team. As a key member of our team, you will be responsible for providing high-quality physical therapy services to patients in their homes.
Responsibilities:
- Assess patients' functional status and physical therapy needs, and develop individualized plans of care in collaboration with our healthcare team.
- Provide physical therapy services within the scope of practice, as defined by state laws, and in accordance with the plan of care.
- Monitor patients' progress, report any changes in their condition to the physician and supervising registered nurse, and adjust treatment plans as needed.
- Set realistic and achievable rehabilitation goals for patients, and guide them in the use of therapeutic activities to improve their function.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
Requirements:
- Current licensure by the Louisiana Physical Therapy Board.
- At least one year of experience as a Physical Therapist.
- CPR certification.
- Excellent time management skills and a patient-centered approach.
- Reliable transportation to visit patients.
- Home Health experience is preferred.
