Option Care Health is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will be responsible for providing safe and appropriate pharmacy services to our patients in accordance with our policies and procedures, as well as applicable regulatory entities.
Key Responsibilities- Assessment and Monitoring: Determine the suitability of individual patients for home care, collect and organize patient-specific information, and assess patients' suitability for home care in accordance with our policies.
- Pharmacy Care Planning: Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for the pharmacotherapy of home care patients.
- Medication Education: Provide medication-use education to our staff, other professionals, home care patients, and/or their caregivers.
- Continuity of Care: Ensure continuity of pharmaceutical care to and from the home and other patient-care settings.
- Medication Preparation and Dispensing: Prepare and dispense medications using appropriate techniques and following our policies and procedures.
- Quality Assurance: Adhere to appropriate safety and quality assurance practices, and prepare labels that conform to our policies and procedures.
- Equipment Management: Manage the use, maintenance, and troubleshooting of medication administration equipment and medication-related equipment used in the management of home care patients.
- Incident Reporting: Participate in the process for assessing, managing, and reporting medication-related incidents and equipment error and/or failures.
- Education and Experience: Graduate of an accredited School of Pharmacy, current, active Pharmacy license as granted by the State Board of Pharmacy, and one of the following: at least one year of experience in sterile IV admixture procedures, knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM), completion of an ASHP accredited Pharmacy Residency Program, completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
- Customer Service: Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction.
- Technical Skills: Basic level skill in Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
- Residency Program: Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
- Home Care Experience: Experience in home care, hospital, or other clean room setting.
- Indirect Management: At least one year of experience in indirect management of team members, including assisting in the development, training, and assignment of work/projects to other team members.
