Your responsibilities will include conducting thorough medical record evaluations and consultations to uncover the medical, social, emotional, and financial requirements of patients dealing with Chronic Kidney Disease (CKD) and End-Stage Kidney Disease (ESKD). You will implement tailored interventions to meet these needs.
In this position, you will operate both independently and collaboratively with a diverse healthcare team to ensure the delivery of high-quality, consistent, and cost-effective care.
Position Highlights:
- Work Environment: Hybrid-Remote, combining telehealth services with occasional travel for in-person patient visits.
- Schedule: Full-time, Monday to Friday, with flexibility to accommodate patient and nephrology partner schedules.
- Build meaningful relationships with patients and their families, fostering trust and understanding.
- Manage comprehensive care for patients, including care transitions and ongoing needs, while collaborating with the care team to establish effective treatment plans.
- Address social determinants of health, including medical, emotional, educational, community, and financial factors impacting patient care.
- Ensure seamless transitions of care when necessary.
- Conduct patient assessments to identify unreported medical conditions or changes that could lead to adverse health outcomes.
Qualifications:
- A strong commitment to providing care for patients with complex, chronic conditions.
- Flexibility and adaptability to changing priorities.
- Current RN License is mandatory; BSN is preferred.
- At least 5 years of nursing experience is required.
- Valid CPR or BLS certification.
- A current driver's license and willingness to travel within a designated area.
- Proficient computer skills, particularly in MS Word, Excel, and Outlook.
- A home office setup with reliable internet connectivity is essential.
Preferred Qualifications:
- 2+ years of experience in Case Management or Chronic Care Management.
- 1+ years of experience in utilization management.
- Proficiency in Spanish is preferred in certain markets.
