Job Summary

We are seeking a skilled and compassionate Home Health RN to join our team at Maximus - Bronx. As a Nurse Assessor, you will play a vital role in providing high-quality patient care and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Conduct thorough nursing assessments on admission and re-evaluate patient needs on succeeding visits
  • Develop and implement individualized care plans in accordance with physician orders and agency policies
  • Manage all therapies, including LPN, PT, OT, ST, and CNA, to ensure comprehensive patient care
  • Prepare clinical and progress notes, as well as summaries of care conferences, in a timely manner
  • Provide education and counseling to patients and their families on meeting nursing and related needs
  • Process orders and notify physicians of patient needs and changes in condition
Requirements
  • Graduate of an accredited school of nursing
  • License in good standing
  • One year of recent experience in Home Health
  • AHA BLS certification
