Nurse Care Manager

Old Colony Elder Services (OCES) is seeking a Registered Nurse (RN) to support our consumers in the community by primarily focusing on Enhanced Community Options Program (ECOP) assessments.
Job Summary:The RN will assess and/or authorize services to be delivered in home by provider agencies to coordinate cost-effective and quality service plans. This includes performing in-home evaluation/clinical assessment to determine consumers' eligibility for programs and services according to procedures and guidelines.

- Performing in-home evaluation/clinical assessment to determine consumers' eligibility for programs and services according to procedures and guidelines.
- Working with the care management team to determine appropriate service options; promote cost-effective service substitutions; establish the frequency, scope, and duration of services; and jointly authorize service packages.
- Current Massachusetts RN License.
- Two years' clinical/community-based experience.
- Extensive knowledge of the medical, emotional, and social needs of elders and people living with disabilities.
- Great assessment and communication skills needed.
- Strong ability to work independently.
- Proficient with Microsoft Office Suite, EMR, and related software.
- Ability to work with an enthusiastic team of like-minded individuals.
