About the Job:

This is a travel contract opportunity in Malden, Massachusetts. The ideal candidate will have at least one year of recent experience working in an SNF setting and be able to manage a treatment plan to promote positive outcomes for patients with diverse diagnoses.

Responsibilities:
  • Develop and implement individualized treatment plans for patients
  • Conduct thorough assessments and evaluations to identify patient needs
  • Provide hands-on therapy and education to patients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
  • Current BLS (AHA Preferred)
  • Active State License
  • Supervisory Professional References
  • Must complete Drug Screen and Background Screen
