Job Summary:
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Leoforce, a leading provider of home health services. As an LPN, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities:
- Provide one-on-one care to patients with a range of health conditions, from 8 to 16-hour shifts.
- Assist patients with transitioning home from the hospital and provide continuous care to help them remain at home with their loved ones.
- Work collaboratively with our team to match nurses with patients based on skill, age, health condition, and acuity level.
- Develop and implement personalized care plans to meet the unique needs of each patient.
