Job Summary

The Licensed Medical Provider in this role works in the Addiction Medicine Clinic, caring for patients suffering from substance use disorders. The provider in this role works as a part of a larger addiction medicine team, including experienced physicians, advance practice providers, PMHNPs, as well as RN's, LPN's, and substance use counselors. The provider performs admission physicals, formulating and documenting a patient-centered plan of care for both management of withdrawal and initiation of maintenance medications to manage substance use disorders.

Key Responsibilities
  1. Screen referrals for potential admission to addiction medicine services;
  2. Conduct histories and physicals on patients within 24 hours of inpatient admission and within 72 hours of outpatient visit, and document assessment and plan in electronic medical record (EHR);
  3. Review nursing assessment done at inpatient admission and issue standing orders;
  4. Issue any additional procedure orders for continuation of medications or initiation of medications needed to address ongoing medical conditions as well as lab orders;
  5. Order necessary labs and review results with patients;
  6. Order and interpret urine toxicology screens, and access the Oregon Prescription Drug Monitoring Database when necessary;
  7. Provide counseling and education on medication options for the treatment of various substance use disorders;
  8. Provide education regarding options for and rationale behind healthy SUD recovery and medical concepts related to addiction;
  9. Formulate and initiate treatment plan with patient's informed consent and participation; Titrates or adjusts medications as needed;
  10. In the inpatient environment, review CIWA/COWS/Vitals results (conducted by Nursing staff) to determine if patient's withdrawal symptoms have stabilized sufficiently for discharge on a day-to-day basis;
  11. Administer medications for SUD treatment via subcutaneous/intramuscular injections, using standard practices including clean technique (training provided);
  12. Use motivational interviewing and active listening to support clients recovering from drug/alcohol dependence;
  13. Address other general health concerns as they arise (to include contraceptive counseling, STI counseling and treatment, routine psychiatric care, emergent/temporary management of issues such as hypertension and diabetes, and treatment of chronic viral infections) and document assessments and medical care in EHR, or refer to primary care, specialty care, or the emergency room as needed.
  14. Complete and sign medical orders (meds, labs, etc.) in a timely fashion;
  15. Review outside health records when necessary;
  16. Document diagnosis, level of service, and procedure codes using EMR;
  17. Provide after hours support to nursing, counseling, and respite staff (when on-call) to address general medical concerns and questions;
  18. Coordinate care both internally (with outpatient counseling, Addiction Medicine clinic, and residential treatment team members) as well as externally (with community PCPs, ER staff, and external AMC clinics) as needed;
  19. Assure quality of care by adhering to therapeutic standards; measure health outcomes against patient care goals and standards, making or recommending necessary adjustments; follow detox and Medical Board philosophies and standards of care set by the state Medical Board, and OAR's for detox programs;
  20. Perform other related duties as assigned and approved by the CMO and BestCare Administration.

Qualifications

Education and/or Experience Requirements:

  • Medical Degree (MD, DO, PA, NP) from accredited university required;
  • Experience in addiction, behavioral health and dual-diagnosis care and treatment required;
  • For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire

Licenses and Certifications:

  • Valid license to practice medicine in Oregon;
  • Current ALS/CPR Certification required;
  • Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization's auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations)

Preferred:

  • Experience working with diverse, multi-cultural communities preferred;
  • Bilingual in English/Spanish a plus

  • Excellent communication skills (oral and written);
  • Strong organizational skills and attention to detail, accuracy, and follow-through;
