Job Title: Wound Care Nurse Job Summary: We are seeking a dedicated and compassionate Wound Care Nurse to join our healthcare team at Accordius Health at St Mary's. The ideal candidate will have extensive experience in providing high-quality wound care to patients in a variety of settings, including nursing homes and workers' compensation environments. Responsibilities: * Conduct thorough wound assessments and develop personalized treatment plans for residents with chronic or acute wounds and prevention methods. * Conduct weekly skin assessments on each resident * Provide skilled nursing care to residents including administering medications and treatments. * Collaborate with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive care. * Implement evidence-based wound care practices * Monitor patient progress and adjust treatment plans as needed to achieve optimal wound healing outcomes as well as preventative methods * Educate residents and their families on wound care techniques, infection control measures, and self-care strategies. * Maintain accurate and detailed records of resident care, including wound assessments, treatment plans, and progress notes. * Participate in quality improvement initiatives to enhance resident outcomes and reduce readmissions. Requirements: * Current license to practice as a Registered Nurse in the state where the position is located. * Bachelor's degree in nursing or a related field. * Minimum of 2 years of experience with a focus on wound care. * Experience working in a nursing home. * Strong knowledge of wound care principles, including infection control. * Certification in wound care is required. * Excellent communication and interpersonal skills. * Ability to work effectively in a fast-paced environment and prioritize tasks to meet resident needs.
