We are seeking a board-certified Family Medicine physician to join our team in Southbury, CT. As a member of our group, you will provide high-quality primary care to patients in a dynamic and supportive environment.
Job Responsibilities:- Examine patients, obtain medical histories, and treat acute and chronic illnesses.
- Order, perform, and interpret diagnostic tests.
- Create treatment plans and involve patients and family members in those plans.
- Initiate specialty referrals and collaborate with management and other physicians to improve care delivery and service.
- Participate in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives.
- Ensure compliance with billing and coding requirements and provide on-call coverage within the primary care group.
- Graduation from an accredited medical school (MD or DO).
- Completion of an accredited Internal Medicine or Family Medicine residency program.
- Valid and unrestricted Connecticut medical license or ability to obtain.
- Valid DEA registration and Controlled Substance certification.
