Job Title: LPN Weekday

Alto Health Care Staffing is seeking a skilled Licensed Practical Nurse (LPN) to join our team at an Assisted Living Facility. As a vital member of our healthcare team, you will provide direct patient care and collaborate with Registered Nurses (RNs) and Certified Nursing Assistants (CNAs) to ensure the well-being and comfort of residents.

Responsibilities:
  • Resident Care:
    • Administer medications and treatments as ordered by physicians.
    • Monitor resident health status, including vital signs, and report any changes to the RN.
    • Assist with resident activities of daily living (ADLs) as needed.
    • Provide wound care, ostomy care, and other treatments as ordered.
    • Participate in resident care planning and implementation.
  • Medication Administration:
    • Prepare, administer, and document medications according to facility policies and procedures.
    • Ensure medication accuracy and safety.
    • Monitor for medication side effects and report any adverse reactions.
  • Documentation:
    • Maintain accurate and up-to-date resident records.
    • Complete incident reports as required.
    • Document medication administration, treatments, and resident assessments.
  • Team Collaboration:
    • Collaborate with RNs, CNAs, and other healthcare professionals to provide comprehensive resident care.
    • Delegate tasks to CNAs as appropriate.
    • Participate in staff meetings and in-service education.
  • Emergency Response:
    • Respond to resident emergencies in accordance with facility protocols.
    • Administer emergency care as needed.
Qualifications:
  • Current LPN licensure in the state of Ohio.
  • Experience in long-term care or assisted living preferred.
  • Strong clinical assessment and problem-solving skills.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively as part of a team.
  • Compassionate and empathetic demeanor.
  • Current CPR certification.
Working Conditions:
  • Fast-paced environment with frequent interruptions.
  • Ability to lift up to 50 pounds.
  • Exposure to infectious diseases.
  • Irregular work hours may be required.
