Job Summary

We are seeking a highly skilled Clinical Care Coordinator to join our team at Uphams Corner Health Center. As a Clinical Care Coordinator, you will play a critical role in coordinating and providing clinical case management services to high-risk patients within our Behavioral Health Community Partner (BHCP) program.

Key Responsibilities
  • Coordinate and provide clinical case management services to high-risk patients
  • Ensure coordinated care and access to Behavioral Health services
  • Participate in problem-solving as a member of the Multidisciplinary Care Team
  • Perform Medication Reconciliations for patients who have completed a Comprehensive Health Assessment
Requirements
  • Minimum Basic Knowledge: Licensed Registered Nurse in Massachusetts
  • Two years of nursing experience, preferably in an ambulatory care or community health setting
  • Experience with case management and care coordination
  • Experience with EMR and related care management registries, databases, and Information Technology/Computer Systems
  • Demonstrated ability to work as an integrated member of a Multidisciplinary Care Team
  • Excellent communication and clinical judgment skills
  • Ability to follow and adhere to care model's operating procedures and standards
  • Experience in a culturally diverse setting
  • Bilingual language skills in English/Spanish, Portuguese, or Vietnamese preferred
