POSITION SUMMARY:
The Care Manager plays a vital role in overseeing patient care, ensuring seamless coordination, and facilitating effective discharge planning. This position is accountable for promoting timely and safe transitions for all patients within their designated caseload.
EDUCATIONAL REQUIREMENTS:
Graduate of an accredited Nursing Program.
EXPERIENCE REQUIREMENTS:
A minimum of two (2) years of experience in either an acute or post-acute healthcare environment.
LICENSURE/CERTIFICATION REQUIREMENTS:
Possession of a current, unrestricted Registered Nurse (RN) license, or eligibility for licensure, in the state of Georgia is mandatory.
PREFERRED QUALIFICATIONS:
Certification in Case Management or Care Management is highly desirable. Familiarity with InterQual or Milliman guidelines is preferred. A Master's degree in a relevant field is also advantageous.
