Job Title: Private Duty Nurse LVN/LPN

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse LVN/LPN to join our team. As a Private Duty Nurse, you will provide one-on-one care to patients in their homes, assisting with daily living activities and promoting independence.

Responsibilities:
  • Implement and update the nursing care plan in collaboration with the patient's physician and healthcare team.
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals.
  • Administer prescribed medications, treatments, and therapies as ordered by the physician.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with the healthcare team to ensure seamless care transitions and address any concerns or changes in the patient's condition.
Requirements:
  • Current, unrestricted state license as a Licensed Nurse (LPN/LVN) in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • One year of experience as a Licensed Nurse (LPN/LVN) in a clinical care setting, preferably in home health or pediatrics.
Preferred Qualifications:
  • Home health experience.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
