Salary
The estimated salary for this position is $34,200 per year, based on national averages and industry standards.

Job Description
In this role, you will be responsible for providing direct patient care services, conducting thorough assessments, and educating patients and families about their care plans. You will also collaborate with multidisciplinary teams to develop and implement individualized patient plans of care, ensuring seamless transitions between care settings and promoting continuity of care. Furthermore, you will be responsible for maintaining accurate and comprehensive patient records and developing strategies to improve patient outcomes.

  • • Provide direct patient care services, including administering medications and treatments
    • Conduct thorough assessments and develop individualized patient care plans
    • Educate patients and families about their care plans and ensure understanding of their conditions
    • Collaborate with multidisciplinary teams to promote seamless transitions between care settings

Required Skills and Qualifications
To succeed in this role, you must possess strong clinical skills, with a focus on patient assessment, critical thinking, and decision-making. You should have experience in medical-surgical nursing, preferably in a progressive care unit or transplant setting. Additionally, you should be familiar with electronic medical records systems and possess basic computer skills.
