Key Responsibilities- Conduct systematic post-acute assessments to determine appropriateness for Home Health and Hospice care settings.
- Inform and educate patients and families about post-acute settings, balancing their requests with safe and reliable ongoing care.
- Identify patient/family problems or needs and communicate with physicians, care management, and the clinical team.
- Assist with coordination of home health care referrals within assigned hospitals.
- Conduct bedside assessments to determine appropriateness of home or hospice care admission and educate patients/families regarding discharge plans and home care/hospice service expectations.
