Social Worker

We are seeking a skilled Social Worker to join our team as a Coordinating Specialist. In this role, you will work closely with clients, colleagues, and other stakeholders to provide needs-based assistance and support social participation in individual and group settings.

• Develop and implement person-centered integrated participation planning (PiT) for clients
• Provide process documentation and evaluation for the team's clients
• Work collaboratively with colleagues to ensure seamless support and services
• Accompany clients on-site and in the Stiftstraße premises
