At Homebase Medical, we are dedicated to delivering high-quality medical care to homebound older adults. Our mission is to optimize their quality of life and enable them to live independently in their own homes.
We offer a range of medical services, including primary care, chronic care management, and referrals to specialist resources. We are backed by SCAN, a not-for-profit organization with over 45 years of experience supporting the health and independence of older adults.
We are currently seeking part-time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our expanding house call practice in Ontario/Fontana areas.
Responsibilities
- Provide primary care services in the home setting, Board and Care, and Assisted Living Facilities.
- Visit patients in their assigned geographic area, driving to locations as needed.
- Average 7-10 patient visits per day (if working a full workday).
- Communicate complex medical information with simplicity and kindness, exhibiting excellent bedside manner.
- Provide health education to patients, working to prevent illness and promote health.
- Refer patients to appropriate medical resources when necessary.
- Utilize our Electronic Medical Record (EMR) system to document patient interactions accurately and timely.
- Demonstrate knowledge of Hierarchical Condition Category (HCC) coding.
- Participate in an on-call rotation, ensuring that patients receive around-the-clock support.
Requirements
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted PA or NP license to practice in California, including DEA license.
- Minimum 1 year of experience in primary care, hospice, or urgent care/outpatient setting preferred.
- Prior experience in a home health, hospice, or other home visit setting is highly desirable.
- Enrollment in Medicare and Keystone credentialing are preferred.
- Strong computer skills, including Microsoft Office and Outlook, are essential.
- Excellent verbal and written communication skills are required.
- Clean driving record, valid auto insurance, and reliable transportation are necessary.
- Bilingual or multilingual abilities, particularly Spanish, are strongly preferred.
