Job Summary

We are seeking an experienced Nurse Practitioner to join our team at Abode Care Partners. As a Nurse Practitioner, you will be responsible for providing compassionate and evidence-based medical care to seniors and patients with complex needs. You will examine and treat illnesses, collaborate with healthcare professionals, and maintain comprehensive patient records.

Responsibilities
  • Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs by performing routine check-ups and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Patient Care Coordination: Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care.
  • Medical Record Keeping: Ensure patient medical records are up-to-date and accurate.
Requirements
  • Licensure and Certification: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills with accountability for practitioner's daily workflow, Proficiency in medical instruments and equipment, Strong knowledge base of HIPAA and OSHA regulations.
Compensation and Benefits

The estimated annual salary for this position is $100,000 - $120,000 per year, depending on experience.
