We're seeking a skilled Advanced Practice Clinician (Physician Assistant or Nurse Practitioner) to join our expanding house call practice. As a key member of our team, you'll primarily perform Medicare Annual Wellness Visits and provide primary care in the home setting, Board and Care, and Assisted Living Facilities.
Responsibilities:- Provide high-quality primary care to patients in their homes, including Board and Care, and Assisted Living Facilities.
- Drive to patient locations in your assigned geographic area.
- Exhibit exceptional bedside manner and communicate complex medical information with simplicity and kindness.
- Provide health education to patients and work with them to prevent illness, promote health, and manage chronic health problems.
- Refer patients to appropriate medical resources and utilize our EMR system to document patient visits accurately and timely.
- Knowledge of HCC coding and active participation in an on-call rotation are required.
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted PA or NP license to practice in California, including DEA license.
- Minimum of 1 year of experience as an Advanced Practice Provider in primary care, hospice, palliative, or urgent care/outpatient setting.
- Previous experience in a home health, hospice, or other home visit setting is highly desired.
- Strong computer skills, including MS Office and Outlook, and a good working knowledge of HCC coding.
