Key Responsibilities
  • Conduct comprehensive assessments and develop individualized treatment plans for patients based on their physical needs and abilities.
  • Provide in-home physical therapy services to patients in accordance with their treatment plans, ensuring a high level of patient satisfaction and outcomes.
  • Continuously assess and adjust treatment plans as needed to ensure progress towards patient goals, collaborating with patients, family members, and other healthcare professionals as necessary.
  • Document all patient interactions, assessments, and treatment plans according to AdvisaCare standards, maintaining accurate and up-to-date records.
  • Adhere to all state and federal laws and regulations related to physical therapy practice, ensuring compliance and maintaining a high level of professionalism.
  • Maintain accurate records and reports of patient progress and treatment outcomes, providing valuable insights for patient care and service improvement.
Requirements
  • Current licensure as a Physical Therapist in the state of Michigan.
  • A minimum of one year of PT experience, ideally in a home health care setting, is preferred.
  • Strong understanding of physical therapy best practices and treatment methodologies, with a commitment to evidence-based care.
  • Excellent communication and interpersonal skills, with the ability to connect with patients and their families, building strong relationships and trust.
  • Ability to work independently and manage a caseload of patients effectively, prioritizing tasks and managing time efficiently.
  • Must have a valid driver's license and reliable transportation, with the ability to travel to patient homes as needed.
