Job Title: Home Health Physical Therapist

We are seeking a skilled Physical Therapist to join our team in Riverhead, New York. As a Home Health Physical Therapist, you will work with patients in their homes, providing personalized care and rehabilitation services.

Job Summary:

This is a 13-week contract assignment with a partnering home health agency. You will work 40 hours per week, with 8-hour shifts, and have the opportunity to earn a tax-free stipend. Our team is dedicated to providing exceptional care and support to our patients, and we are looking for a Physical Therapist who shares our commitment to excellence.

Responsibilities:
  • Provide physical therapy services to patients in their homes, including assessments, treatments, and education.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Document patient progress and maintain accurate records.
Requirements:
  • Graduate of an accredited Physical Therapy program.
  • Licensed as a Physical Therapist in New York.
  • CPR certified or ability to obtain.
  • Previous experience with computer charting.
  • 6 months of previous home health care experience.
  • Valid U.S. driver's license and reliable transportation.
