Key Responsibilities:

What to Expect as a Matrix Provider:

  • Perform comprehensive Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and appropriate referrals.
  • Collaborate effectively with physicians, case managers, social workers, family members, and other relevant healthcare personnel.
  • Engage with Primary Care Physicians (PCP) for patient education and follow-up care.
  • Deliver services across various settings, including home visits, skilled nursing facilities, and telehealth consultations.

Position Details:

Type: PRN/Part-Time - Flexible Schedule

Compensation: $90.00 or $115.00 per completed assessment, depending on county

Location: Various community environments

Schedule: Flexible hours including days, evenings, nights, and weekends

Benefits: Non-benefited position with access to 401(k) and limited voluntary benefits, along with a mileage reimbursement plan.


Qualifications:

Essential Requirements:

  • Master's Degree or equivalent experience with successful completion of NP licensure.
  • Master's Degree or equivalent experience with successful completion of PA licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescriptive authority.
  • Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant, with the ability to travel as needed.
  • Proficient computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • A positive, adaptable personality and a collaborative team spirit.

Travel Requirements:

  • Valid state driver’s license and the ability to drive, with proof of adequate automobile insurance.
  • Travel required to provide care in patients' homes.
