Job Summary

Care Around the Block is seeking a skilled Healthcare Outreach Specialist to provide population health services to assigned clients on an ongoing and emergency basis. The ideal candidate will have a minimum of 3 years experience in local human service or medical field, as well as resources regularly utilized by older adults in their community.

Key Responsibilities

  • Provide population health services to assigned clients on an ongoing and emergency basis per contracted intervention protocol, within contracted timeframe, by telephone.
  • Develop and maintain an accurate and thorough knowledge of all program requirements.
  • Develop and maintain skillset on tools required to perform job tasks in timely and accurate manner including specialized computer software.
  • Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies.
  • Communicate with supervisor on daily basis, and as needed, to report all areas of concern/issues/incidents to department supervisor.

Requirements

  • A minimum of 3 years experience in local human service or medical field, as well as resources regularly utilized by older adults in your community.
  • Training in healthcare and/or customer service position utilizing advanced technology tools including customized software systems.
  • Computer proficiency in Microsoft Word, Excel, and Outlook.
  • Ability to pass a Criminal Background Check, Drug Screen, and DMV Check at hire and randomly as required by company policy.

Working Conditions

This is a hybrid, work-from-home/Field Assessment position based in Chattanooga, TN. Non-Satellite internet connection is a requirement for the job.
