We are seeking a skilled LPN to join our Ophthalmology team at Kaiser Permanente. As an LPN, you will play a vital role in delivering high-quality patient care and contributing to a positive care experience and work environment.
Key Responsibilities:- Participate in delivering quality and customer-focused care to patients in a manner that reflects Kaiser Permanente's organizational and nursing mission, vision, and values.
- Provide nursing and administrative support to clinicians and other healthcare team members to offer a variety of healthcare services to patients.
- Perform clinical tasks and procedures under the direction of a clinician, within the scope of the LPN's professional training and individual competency.
- Work under the clinical supervision of the clinician or registered nurse and under the general direction of the designated supervisor.
- Prepare and organize patients' office visits for known reasons/procedures by ensuring that all appropriate tests, clinical information, and order entries are ready for clinician review prior to appointment time.
- Perform focused/limited nursing assessment and triage on routine or self-limiting medical conditions as directed by RN/clinician.
- Recognize signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing, and implement planned nursing and medical interventions as delegated by RN/clinician.
- Contribute to the development, implementation, or modification of the individualized patient's plan of care under the direction of the RN/clinician.
- Maintain and adjust clinicians' patient schedule for efficient patient flow that facilitates appointed visits and overbooks.
- Answer and respond to incoming messages/requests via telephone, mail, or from patients or other departments by collecting and communicating pertinent data requiring clinicians' review and input.
- Facilitate and complete follow-up duties on patients' plan of care as directed by clinician to include but not limited to: processing of prescriptions and forms; reminder of screening/diagnostic tests that are due; scheduling/staging of procedures, tests, referrals, and/or return appointments; and obtaining and inputting patient data to monitor progress into the electronic medical record.
- Graduate of an accredited school of nursing.
- Practical Nurse License (Washington) within 6 months of hire OR Compact License: Practical Nurse within 6 months of hire.
- Practical Nurse License (Oregon) within 6 months of hire.
- Basic Life Support.
- Two (2+) years of nursing experience in assigned departments' clinical area within an ambulatory care setting.
- IV therapy experience or recent certification.
- Familiarity in documenting in an electronic medical record system.
