Job Summary

CVS Health is seeking a skilled RN Case Manager to join our team in a field-based position. As a key member of our team, you will be responsible for assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs and facilitate their overall wellness.

Key Responsibilities
  • Conduct telephonic and face-to-face assessments to evaluate member needs and develop proactive courses of action to address issues presented.
  • Develop and implement case management plans to enhance short and long-term outcomes and opportunities for member wellness.
  • Collaborate with members, healthcare providers, and other stakeholders to ensure seamless transitions to Aetna programs and plans.
  • Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address complex health and social indicators impacting care planning and resolution of member issues.
  • Utilize case management processes in compliance with regulatory and company policies and procedures.
Requirements
  • Minimum 3 years of clinical practice experience required.
  • Current Active unrestricted RN licensure in the state of IL required.
  • Willing and able to travel 50-75% of their time to meet members face-to-face in Winnebago County, IL and surrounding areas.
  • Reliable transportation required; mileage is reimbursed per company expense reimbursement policy.
  • Minimum 1 year experience in MS Office suite applications.
Preferred Qualifications
  • Case Management in an integrated model preferred.
  • Experience working with adults with Behavioral Health needs preferred.
  • Long Term care experience in Nursing Home setting preferred.
