Job Responsibilities

The Geriatric Home Health Professional will be responsible for assessing client needs and developing personalized plans to meet those needs. This will involve:

  • Evaluating client goals and objectives
  • Coordinating durable medical equipment and supplies
  • Providing or arranging assistance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Providing or arranging respite care to reduce caregiver stress levels

The ideal candidate will have experience working with geriatric populations and possess strong clinical skills, including medication management and fall prevention strategies.

A current active Practical Nurse License (LPN) in the State of Michigan is required, as well as 3 years of experience as an LPN in a geriatric setting. Reliable transportation and a valid Michigan driver's license are also essential.
