Qualifications:
- Possession of a valid Nursing Assistant License in the State of Vermont.
Requirements:
- High School Diploma or equivalent.
- Valid Driver's License and current auto insurance.
- Open to both experienced and newly graduated CNAs.
- Must be at least 18 years old.
- Availability for weekend and holiday shifts is necessary.
Key Responsibilities:
- Under the guidance of a Registered Nurse or licensed Therapist, receive daily patient assignments and execute care plans tailored to individual patient needs.
- Gather patient and family information and communicate findings promptly to the RN or licensed Therapist.
- Facilitate effective communication with assigned patients.
- Perform designated interventions and procedures under the supervision of an RN or licensed Therapist to ensure high-quality patient care.
- Report relevant patient and family information to the RN or licensed Therapist.
- Advocate for patient care requirements to ensure that the quality and type of care are responsive to each patient's unique needs.
- Carry out various patient care tasks, including assistance with daily living activities, monitoring vital signs, and supporting mobility and exercise.
- Act as a Hospice Aide when directed.
- Function as a Personal Care Attendant as needed.
- Timely report any changes in skin conditions to the RN, Case Manager, or program manager.
