Key Responsibilities:
  • Provide comprehensive diabetic counseling and education to patients, including one-on-one and group sessions.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Administer medications and treatments as prescribed by physicians.
  • Maintain accurate and detailed patient records, including medical histories and treatment plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
  • Graduate of an accredited school of nursing with a current Registered Nurse (RN) license in the State of Ohio.
  • Certified Diabetes Educator (CDE) certification required within 1 year of hire.
  • 2 years of experience as an RN in a physician's office or hospital setting preferred.
  • Knowledge of specific physician specialty preferred.
  • Exceptional communication, telephone etiquette, customer service, and organizational abilities.
  • Proficiency in managing a fast-paced environment and multitasking.
  • Basic computer skills required.
  • Medical terminology, anatomy, lab, instrument sterility, sterile technique, phlebotomy, and clinical skills required.
Working Conditions:

This is a full-time position requiring a flexible schedule to meet the needs of our patients.
