Director of Care Coordination

The Director of Care Coordination will play a pivotal role in shaping the strategic vision of the care management program at Wellstar Health System, Inc. This individual will work closely with the Chief Medical Officer, Chief Financial Officer, and other senior leaders to develop and implement strategies that align with the hospital's goals and vision. Key responsibilities include building and maintaining relationships with physicians, analyzing data to drive departmental performance, and ensuring adherence to system utilization management plans and care coordination policies.

• Develop and implement strategies to improve care coordination and management processes
• Collaborate with physicians and other departmental leaders to address barriers to care management outcomes
• Analyze data to drive performance improvement initiatives and ensure alignment with targeted goals
• Manage and interpret data to inform decision-making and drive departmental performance
• Recruit, develop, and retain high-performing staff, including mentoring, coaching, and performance appraisals
• Participate in committees to address and solve issues that drive variation from goals

• Bachelor's degree in Nursing or Master's degree in Social Work required
• Master's degree in Public Health or Health Administration preferred
• Minimum 3 years of management experience in case management or quality and patient safety
• Minimum 6 years of experience in a hospital environment
• Strong supervisory experience and clinical practice/experience in a licensed field
