- Develop and implement personalized care plans to achieve patient goals and improve functional status
- Provide skilled physical therapy interventions to support patients throughout the continuum of care
- Instruct patients and caregivers on therapeutic exercises, appliances, and devices to promote independence and safety
- Prioritize caseload and coordinate daily staffing needs to ensure efficient and effective care delivery
- Licensed Physical Therapist with a Bachelor's degree
- Valid driver's license
- Occasional/intermittent required
