As a PRN Registered Nurse, you will provide patient-centered care in the home, focusing on comfort, dignity, and support during life's most challenging moments.

Responsibilities:

* Conducting Start of Care comprehensive assessments
* Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
* Preparing accurate, orderly, and timely documentation
* Coordinating care with patient and provide proper reports to Case Manager
* Provides health care instructions to the patient/family as appropriate per assessment and plan

Requirements:

* An active RN license in the state in which practicing (Minimum of 1 year of experience preferred)

* Excellent communication and interpersonal skills
* Compassionate and empathetic approach to patient care
* Ability to work independently and as part of a team
* Knowledge of hospice regulations and standards of care
* Reliable transportation and willingness to travel
