Job Summary

We are seeking a skilled and compassionate Weekend Registered Nurse to join our Home Health team at St. Luke's Hospital. As a Weekend RN, you will provide high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Key Responsibilities
  • Evaluate and perform comprehensive assessments on patients referred for home health nursing services.
  • Determine eligibility for admission, obtain financial and statistical data, and formulate initial care plans according to patient needs.
  • Refers patients to other agency disciplines and community resources as needed.
  • Assesses, promotes, and maintains health of individuals, families, and communities through teaching, counseling, and preventive measures.
  • Provides ongoing assessment to determine service frequency, change in care plans, and discharge from service.
  • Functions as the designated coordinator of patient care, ensuring provision of optimum, comprehensive multidisciplinary care.
  • Communicates effectively with patients, families, physicians, and related persons, exercising tact, initiative, and good judgment in dealing with people.
Requirements
  • A graduate of an approved school of professional nursing, licensed as a Registered Nurse by the state in which practicing, with a Baccalaureate degree preferred.
  • Two years of experience as a professional nurse in nursing practice or previous community health experience as a professional nurse preferred.
