As a Case Manager FIS, you will play a critical role in providing services to enable individuals and families to access needed medical, social, educational, and other support services.

Your primary responsibilities will include assessing, advocating, linking, and coordinating needed community-based services for clients and families served. You will work closely with clients and their families to promote their educational, legal, vocational, extra-curricular, and health-related goals.

**Key Responsibilities:**

* Provide services to enable individuals and families to access needed medical, social, educational, and other support services
* Assess, advocate, link, and coordinate needed community-based services for clients and families served
* Work closely with clients and their families to promote their educational, legal, vocational, extra-curricular, and health-related goals
* Meet with clients and their families in their homes or residences, as well as in community locations such as schools, daycare centers, and job sites
* Develop and maintain relationships with community partners and resources to ensure seamless service delivery

**Requirements:**

* Bachelor's degree in social sciences or related field
* 1 year of relevant professional experience working with families and children
* Training in proper crisis prevention initiatives and universal precautions
* Proof of automobile insurance and reliable transportation
* Valid driver's license
