The Hospice Registered Nurse Case Manager is responsible for overseeing and coordinating hospice care, emphasizing community health education and experience. This role requires a skilled nursing professional who can leverage community resources to effectively address the needs of patients and their families in their homes.
Key Responsibilities
Patient Care
- Conduct comprehensive assessments of patients and families to identify hospice needs, including a thorough physical evaluation and medical history.
- Deliver professional nursing care by applying all aspects of the nursing process.
- Regularly assess and evaluate the patient's condition, initiating and adjusting care plans as necessary.
- Develop individualized care plans that set clear goals based on nursing diagnoses, incorporating palliative care strategies.
- Administer medications and treatments as prescribed, while providing education and support to patients and families.
- Assist patients with daily living activities, promoting self-sufficiency and comfort.
- Act as the RN Case Manager for assigned patients, ensuring coordinated care.
Communication
- Maintain accurate clinical documentation regarding patient care and condition.
- Collaborate with physicians to address patient needs and report any changes in condition.
- Engage with community health professionals to ensure cohesive care delivery.
- Educate patients and families on self-care techniques and health management.
- Foster a safe environment for patients and their families.
- Participate in interdisciplinary team meetings to support emotional and psychological needs.

- Participate in on-call duties as per policy guidelines.
- Ensure availability of necessary equipment and services for patient care.
- Supervise and delegate tasks to ancillary staff as needed.
