About This OpportunityThis LPN/LVN position is located in an outpatient clinic setting where you will be responsible for providing high-quality patient care to our nation's veterans. You will work closely with our multidisciplinary team to assess patient needs, develop and implement individualized care plans, and provide education and support to patients and their families.
Key Responsibilities- Assess patient needs and develop individualized care plans
- Provide direct patient care, including administering medications, performing treatments, and documenting patient information
- Work collaboratively with other healthcare professionals to ensure seamless care delivery
