Job Description

We are seeking a Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN, you will be responsible for providing individualized goal-directed nursing care through the use of the nursing process in accordance with physician's orders.

Responsibilities
  • Provide quality care to assigned patients
  • Carry out the plan of care for each patient, ensuring that the patient/care givers are included in the care plan process
  • Ensure documentation is completed, communicating via a note the activities and needed actions of each visit to the RN
  • Implement medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients
  • Evaluate a variety of aspects of implemented plan of care, including identified problems, care provided, and patient's responses, to meet patient/care giver needs and ensure overall quality of care delivered
  • Maintain timely and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure on-going and continued quality of care
  • Perform patient and care giver education to ensure informed patient decisions can be made, including medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services
  • Appropriately identify inter-disciplinary needs of the patient and document patient needs accordingly, and communicate recommendations to case manager in a timely manner
  • Carry out visits in a time-effective manner, taking action to ensure that visits are not combined with routine visits whenever necessary
  • Transmit patient incidents reports (infections, injuries, etc) to QI Specialist in a timely manner
  • Coordinate a wide variety of patient care with physicians, providers, team members, vendors, etc. in support of the recommended plan of care to ensure safe and effective patient and family care
  • Pro-actively advocate for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs
Qualifications
  • Licensed Practical Nurses (LPN) license
  • Prior clinical acute care nursing experience required. Prior Home Care Experience preferred
  • Valid Driver's License, Vehicle and Insurance. Ability to successfully pass motor vehicle record check, child abuse, criminal record check, and adult abuse check required
