The Licensed Practical Nurse (LPN) will participate in the implementation and evaluation of patient/resident care for stable and predictable patients/residents of all ages in the home care setting.
Responsibilities- Participate in the implementation and evaluation of patient/resident care
- Assess data reflective of the patient's/resident's status and interpret appropriately
- Provide care needed to ensure the health, comfort, and safety of patients/residents
- Administer a variety of treatments and medications as directed
- Document and maintain records
- Establish and maintain effective working relationships with patients/residents, health care providers, and the public
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
- Obtains and subsequently maintains required department specific competencies and certifications
