Job Summary:

The Licensed Practical Nurse - Patient Care Specialist utilizes advanced knowledge of physiological, psychological, and spiritual principles to deliver high-quality patient care. As a key member of the multi-disciplinary team, this role requires the ability to work independently and execute a workload with minimal supervision.

Key Responsibilities:

  • Delivers selected treatments, procedures, and tests under physician orders, while supervised by the RN.
  • Provides basic patient care on an intermittent basis, demonstrating knowledge of the principles of growth and development, as well as the physical, emotional, and psychosocial needs of the patient population served.
  • Accountable to the Care Manager in relation to utilization practices and to the PI/Education Coordinator in relation to regulatory and compliance standards.
  • Must be able to use and operate medical and/or office equipment, including Point of Care devices.
  • Expected to take night and weekend calls, and make home visits and/or work in office during the evening, night, weekend, and/or vacation hours as required.

Requirements:

  • Currently licensed as a Licensed Practical Nurse in the state(s) in which they are practicing.
  • Certification in CPR prior to patient care.
  • Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty.
  • Maintains a valid driver's license.
  • Demonstrates ability to work independently and execute own workload.

Preferred Qualifications:

  • One-year recent nursing experience.
  • Home health care experience.
