As a Physical Therapist, you will be responsible for providing high-quality rehabilitation services to patients in their homes. Our company is committed to delivering exceptional patient care and customer service, and we are looking for a dedicated professional to help us achieve this goal.

Responsibilities:
  • Evaluate and develop comprehensive physical therapy plans for patients in accordance with their Plan of Treatment.
  • Administer physical therapy modalities and regimens to patients in their place of residence.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes accurately and thoroughly.

Requirements:
  • Current PT licensure in the state of Georgia.
  • Excellent communication and documentation skills.
  • Experience in Home Health is preferred but not required.

Work Environment:
