Job Summary
We are seeking a compassionate and dedicated RN Clinical Liaison to join our Admissions team at Hospice of the Chesapeake. As a key member of our team, you will serve as an intermediary liaison between contracted hospitals and our hospice team, making inpatient visits to hospice patients, acting as a consultant for pain and symptom management, providing in-service education to providers, and performing pre-admission evaluations for appropriateness of hospice services.

Main Responsibilities:
  • Provides admission assessment and engages with patients to develop rapport and trust.
  • Collaborates with patients and families to develop a plan of care, adjusting communication style as necessary.
  • Establishes priorities for patient care based on immediate patient condition and mutually set patient needs/expectations.
  • Performs consent visits to provide information regarding HOC services and obtains signed consents.
  • Communicates relevant information to providers, including any changes in referral diagnosis.
  • Case manages patients who are GIP level of care, ensuring they receive daily Skilled Nursing Visits.
  • Conducts death visits and assists in assessing family needs for bereavement services following patient death.
  • Assists with community admissions when there are no referrals or GIP patients at prospective hospitals.

Qualifications:
  • Current Maryland RN license or accepted reciprocal out-of-state nursing licensure; BSN preferred.
  • 2 or more years of nursing experience in med/surg or home care required, with experience in hospice, end-of-life, or critical care experience preferred.
  • Hospice certification (CHP) preferred.
  • Strong and creative problem-solving skills with the ability to prioritize and delegate.
  • Familiarity and comfort with documenting in an Electronic Medical Record.
  • Valid driver's license with an acceptable driving record and reliable transportation.
  • Ability to meet the physical demands of the position with or without reasonable accommodation.
