Job Summary

This role involves providing clinical social work services to identified patients and families, collaborating with other healthcare professionals as part of a multi-disciplinary team, and facilitating communication regarding relevant psychosocial issues. As a key member of our team, you will function as a coordinating link between patients, families, hospitals, and community resources to ensure continuity of care.

Responsibilities
  • Provides clinical social work services to identified patients and families
  • Collaborates with other healthcare professionals as part of a multi-disciplinary team
  • Facilitates communication regarding relevant psychosocial issues through recording in medical records, consultation, and participation in meetings
  • Serves as a coordinating link between patients, families, hospitals, and community resources to assure continuity of care
  • Participates in multidisciplinary quality and service improvement teams as appropriate
Requirements

A Master's degree in Social Work from an accredited training program is required. Experience working with adults and children is preferred.

Compensation and Benefits

The estimated salary for this position is $70,000 - $90,000 per year, depending on experience. Additionally, we offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
