Primary Care Nurse Practitioner Opportunity at a Community Health Center

Harbor Health Services, Inc. is seeking a dedicated and skilled Family Nurse Practitioner to join our team at the Harbor Community Health Center in Plymouth, MA.
Estimated Salary: $80,000 - $110,000 per year
About the RoleWe are looking for a compassionate and patient-focused individual who can provide high-quality primary care to our patients. As a Family Nurse Practitioner, you will be responsible for delivering routine health maintenance, managing chronic conditions, and addressing acute illnesses. You will work closely with our multidisciplinary team to ensure seamless care coordination and excellent patient outcomes.
Responsibilities:
- Provide routine health maintenance, including interval well visits, complete histories, physicals, developmental assessments, risk assessments, cancer screenings, and health promotion education.
- Manage common chronic illnesses, including acute complications and routine follow-up, providing or arranging for appropriate patient education regarding self-management of chronic illnesses.
- Participate in an on-call system with other providers to ensure 24-hour access to care for our patients.
- Collaborate with our care team to develop and implement individualized care plans that meet the unique needs of each patient.
- Stay up-to-date with the latest clinical guidelines and best practices, applying evidence-based medicine to inform your decision-making.
To be successful in this role, you must have:
- A completed accredited Nurse Practitioner program in Family Practice.
- A current MA NP License, Board Certified, Current DEA, MCSR, and Basic Life Support for Healthcare Providers (BLS) required.
- 3-5 years of NP experience preferred, with a strong track record of delivering exceptional patient care.
- Experience working in a community health center setting and with a diverse patient population is highly valued.
- Bilingual skills are a plus.
