This is a unique opportunity for a skilled Nurse Practitioner to join our team at Barnabas Health Medical Group. As a Palliative Care Nurse Practitioner, you will play a vital role in providing high-quality patient care and collaborating with our multidisciplinary team to ensure the best possible outcomes.
Responsibilities- Assess patient needs and develop individualized care plans in collaboration with physicians and other healthcare professionals.
- Provide primary health care through assessment, diagnosis, and management of patients with complex medical conditions.
- Order and review laboratory and diagnostic procedures, and interpret results to inform patient care decisions.
- Prescribe medications and treatments as indicated, and educate patients and families on medication management and treatment instructions.
- Collaborate with physicians and other healthcare professionals to coordinate patient care and ensure continuity of care between visits.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure high-quality patient care.
- Master's degree in Nursing required.
- Registered Professional Nurse with current NJ state license; CDS and DEA.
- CPR certification.
- NP Certification.
- Three to five years of work experience as a professional nurse.
This is a per diem position, working days. The ideal candidate will be able to work independently and as part of a team, and will have excellent communication and interpersonal skills.
