Qualifications for MDS Coordinator - RN or LPN:
  • Valid RN or LPN license in Colorado or a reciprocal state
  • CPR certification required
  • Previous supervisory experience preferred
  • Ideally, 1 year of experience as an RN or LPN
Responsibilities of MDS Coordinator - RN or LPN:
  • Assist the Director of Nursing with daily supervisory responsibilities
  • Collaborate in the preparation of accurate and timely multidisciplinary MDS Assessments and Care Plans
  • Ensure compliance with state and federal regulations
  • Adhere to established protocols and policies of the facility
