Job Title: Physical Therapist/Physical Therapy Assistant

About the Role:

We are seeking a skilled Physical Therapist or Physical Therapy Assistant to join our dynamic care team at Bayview Health Care in Waterford, CT. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of our residents and contribute to our mission of delivering exceptional healthcare services.

Responsibilities:

  • Evaluate patients/residents and develop appropriate care plans.
  • Provide direct resident treatment in accordance with an established plan of care.
  • Contribute to the team meeting and decision-making process as defined by the program.
  • Consult with and train patients, residents, and caregivers.
  • Monitor efficiencies on an ongoing basis in collaboration with supervisor.
  • Document activities according to the company's clinical documentation system on a timely basis.
  • Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations.
  • Communicate effectively with patients, residents, team members, customers, and supervisor.
  • Provide appropriate consultation to the facility.
  • Participate in clinical program development and implementation.
  • Participate in the training and orientation of other staff, students, and volunteers.
  • Participate in the interview process for new staff.
  • Participate in census building and marketing activities.
  • Participate in quality improvement activities.

Requirements:

  • Ability to function effectively in multi-faceted systems and to interact effectively with all levels of staff and customers.
  • Strong oral and written communication skills.
  • Strong organizational and analytical skills.
  • Ability to perform documentation needs, manipulations of equipment, and direct patient care.
  • Requires intense attention and concentration.
  • Ability to cope with periodic stress associated with changing census and staffing demands.
  • Demonstrated ability to prioritize and manage time effectively for self and others.
  • Ability to deal with a wide range of personality types, ages, and diverse cultural backgrounds.
  • Ability to articulate and comprehend the English language.
  • Standing and walking for significant amounts of time is necessary.
