Adoration Home Health and Hospice is seeking a dedicated Physical Therapist to join our team in providing quality care to patients in the comfort of their own homes.
About the Role:- Perform physical therapy evaluations and treatments based on physician orders, including recommending and procuring adaptive equipment.
- Establish measurable goals and develop treatment plans to help patients achieve their maximum level of independence and function.
- Maintain open communication with the treatment team, physician, patient, and family members as needed.
- Document services provided in a thorough and timely manner to ensure proper billing.
- Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities.
- Minimum Bachelor's Degree in Physical Therapy from an accredited program.
- Current, unrestricted license as a Physical Therapist in the state of operation.
- Current BLS or CPR certification in accordance with state regulations.
- Minimum one year of PT experience preferred.
