We are seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team at Lynn Community Health Center. As a key member of our healthcare team, you will provide high-quality patient care, education, and support to our diverse patient population.
Responsibilities- Provide comprehensive primary care services to patients of all ages, including health assessments, diagnosis, and treatment of acute and chronic conditions.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with our multidisciplinary team, including physicians, nurses, and behavioral health providers, to ensure comprehensive and coordinated care.
- Participate in ongoing education and professional development to stay current with best practices and evidence-based guidelines.
- Contribute to the development and implementation of quality improvement initiatives to enhance patient outcomes and satisfaction.
- Board Certification/Board Eligibility in Family Medicine
- Massachusetts license for the practice of medicine
- CPR certification
- Bilingual in English and Spanish preferred
